When Your Phone Becomes a Burden for Shoulders, Joints and Spine

In an age where the smartphone is our constant companion: what happens to the neck, shoulders and spine when you're always looking down at a screen?

In an age where the smartphone is our constant companion, we see it everywhere: people with their heads down, eyes fixed on a screen. What looks harmless at first glance has far-reaching consequences for the musculoskeletal system. And it's not just the neck and spine that take the hit — the shoulder joint does too, an aspect that's frequently underestimated.

The more the head tilts forward, the more the load on the cervical spine rises exponentially: 0° tilt – around 5 kg of load, 15° tilt – 12 kg, 30° tilt – 18 kg, 60° tilt – 27 kg.

The Silent Overload: What Happens Inside the Body

1. Chronic muscle tension in the shoulder-neck complex. Constantly bending the head forward keeps the neck and shoulder muscles permanently active: trapezius, levator scapulae, rhomboids. The muscles harden, which can lead to pain, restricted movement and tension headaches.

2. Overload of the cervical spine. The cervical vertebrae have to absorb the extra weight — which can lead to premature disc wear, compress nerves (cervical radiculopathy), or trigger chronic cervical spine syndromes.

3. The shoulder joint under pressure – risk of impingement syndrome. What many people overlook: the hunched posture also has a negative effect on shoulder mechanics. Constantly pushing the shoulders forward narrows the subacromial space, and the supraspinatus tendon rubs against the acromion more and more. That sustained stress promotes impingement syndrome.

4. Effects on the thoracic spine. The chest muscles shorten while the upper back muscles get overloaded. A lasting imbalance leads to postural damage and restricted breathing.

What Can You Do?

Hold your phone at eye level. Take a movement break every 30 minutes. Build shoulder stabilization exercises into your day — external rotations, stretches for the chest and neck. And check your posture regularly.

The digital world has its price. Counteract it early and you protect not just your spine, but the healthy function of your shoulders as well.
